Travel Document

TRAVEL DOCUMENT FOR ADULTS

The following documents must be presented at the Consulate General: 

1. Submit an e-application at https://geoconsul.gov.ge/ (In order to receive the travel document by mail, please indicate "postal service" when submitting e-application.)
2. Application form must be filled out in Georgian language (only necessary when requesting the travel document by mail) .
3. Copy of the applicant’s passport (if applicable);
4. A money order in the amount of $35 U.S. dollars written out to the Consulate General of Georgia in SF;
5. 1 passport size photo (3.5 x 4.5 cm).

The documents mentioned above can be submitted in person at the Consulate General or sent by mail. Documents sent by mail, must be accompanied by a prepaid envelope with a return address.

TRAVEL DOCUMENT FOR MINORS

The following documents must be presented at the Consulate General: 

1. Submit an e-application at https://geoconsul.gov.ge/ (In order to receive the travel document by mail, please indicate "postal service" when submitting e-application.)
2. Application form must be filled out in Georgian language (only necessary when requesting the travel document by mail). The application is filled out by the parent. Point 12 is signed by the parent of the child aged 0-13. If the child is aged 14 and up, the child signs point 12. Point 13 should be filled out and signed by the parent.
3. Copy of child’s passport (if applicable);
4. Copy of parent's (Applicant's) passport;
5. Copy of birth certificate (if born outside of Georgia - should be presented with an apostille and Georgian notary translation);
6. 1 passport size photo (3.5 x 4.5 cm).

The documents mentioned above can be submitted in person at the Consulate General or sent by mail. Documents sent by mail, must be accompanied by a prepaid envelope with a return address.
